Sutton and Merton primary Care Trust (PCT) employs experienced nurses who help patients to live independently in their own homes and enjoy a better quality of life - reducing the need for urgent medical treatment at hospital

Margaret, 78, a local resident from North Cheam who has benefited from the scheme, commented:

"I suffer with angina and get very anxious - I used to end up in hospital regularly.  Now, my case manager, Jackie, visits every week and my physiotherapist, Christine, visits to show me exercises to help my mobility, angina and breathing when I get anxious.  We manage my problems before I get bad and have to go to hospital - I think this service is really brilliant, I feel so much more confident now."

Joe, 91, Margaret's partner added:

"Having this extra support has taken a load off my mind.  I dealt with all Margaret's medication which was a real worry, but now we receive all the medication in packs which help her take the right tablets at the right time.  It is such a help.
